The approval process for Merch By Amazon currently can take anywhere from a couple of weeks to a few months. The time it takes to get approved depends on a number of factors, including the number of applications submitted, and the backlog of applications already waiting on approval.
This is a lot quicker than it used to be. It took me over 6 months to get approved! Amazon didn’t even bother to send me an email to let me know I’d been accepted. I just happened to check one day and noticed I’d been approved.

What you can be doing in the meantime while you wait to be approved is getting designs done and uploading them to sites like Etsy, Teespring, Redbubble, and TeePublic as did while waiting. Doing this meant I was already making a few hundred dollars a month while I wait to be approved by Amazon.
